THIAMINE AND THE ELDERLY ORTHOPAEDIC PATIENT

An assessment of thiamine status was made in two groups of elderly orthopaedic patients. None of the 30 men and women presenting for elective total hip replacement was malnourished before operation but there was a significant fall in the thiamine status 48 hours later, which had returned to normal by 14 days. In contrast, the majority of the 34 patients in the second group were thiamine deficient after surgery for a femoral neck fracture and remained so throughout the initial post-operative period of 14 days. In both groups those patients who were noticeably confused after operation were found to have a considerable fall in their thiamine status suggesting this may be a contributory factor to postoperative confusion.

THE PREDICTION OF BODY POTASSIUM IN THE ELDERLY

Whilst there are many regression formulae in the literature from which a predicted or expected value of body potassium can be calculated, different formulae give widely differing results. Several of these algorithms have been developed by estimating body potassium from exchangeable measurements which are not valid in most clinical situations. Those derived from studies of direct body-potassium measurement from whole-body counting have included few females or older persons—the very group likely to suffer disease-produced changes in potassium homoeostasis. Eleven male and 11 female carefully screened healthy adults (mean age 75, range 65–90 years) have been used to develop new formulae. It is shown that within this age range body potassium can be predicted from body weight to within ± 20.4% in males and ±15.1% in females. These new relationships should be used in clinical practice for elderly patients.

A study of 20 asymptomatic elderly people living at home or in residential care and 20 symptomatic elderly subjects (with falls, ‘collapse’, dizziness, ‘funny turns’ etc.) revealed that cardiac arrhythmias are common in both groups. Only three of the asymptomatic group and two of the symptomatic group had sinus rhythm throughout their 24-hour electrocardiographic recordings. In the symptomatic group, the arrhythmias bore no relation to the symptoms. Clinical follow-up of up to 6–9 months revealed that elderly people with rhythm abnormalities noted on 24-hour electrocardiographic recording do not develop symptoms.

Echocardiographic studies were undertaken in 15 patients with acute blood loss and 15 patients with chronic anaemia, before and after blood transfusion. Clinically adequate blood transfusion resulted in a 5 mm increase in mean left ventricular internal dimension at end systole in those with acute blood loss and a 3 mm increase in those with chronic anaemia. None of these patients developed acute pulmonary oedema. This study provides base-line data for changes in ventricular dimension resulting from the standard medical management of patients with acute and chronic blood loss. Functional aortic valvular insufficiency was documented in some cases and pericardial effusion resolving after blood transfusion was common.

In recent years increasing interest has centred on the elderly psychogeriatric patient living in the community and the part played by relatives in supporting these patients. There is a need, however, for ways of assessing the behavioural disturbance shown by such patients at home and the effect this behaviour has on relatives. Ratings by relatives of the behaviour at home of elderly dementing patients attending a geriatric psychiatry day hospital, together with the relatives' own ratings of the degree of stress and upset being experienced were obtained. Using the technique of factor analysis it was shown that the patient's behaviour and the relative's reaction could be analysed into a number of separate categories and that these were differentially related to each other. Thus, for example, personal distress in the relative was related mainly to the amount of apathetic and withdrawn behaviour shown by the patient, whereas negative feelings held by the relative towards the patient were related only to the degree of disturbance of the patient's mood. The construction of scales measuring these different aspects of patient's behaviour and relative's reaction is described.

Phenol lumbar sympathectomy for ischaemic lower limb was performed in 38 elderly patients. Over a follow-up period of up to 30 months, excellent results were obtained in 18 patients, all of whom experienced total relief from pain and healing of gangrenous ulcers of the toes. Five patients showed a temporary improvement. In 15 patients the results were poor.

The hypothesis is formulated that ageing is mediated through alteration in a pacemaker system located in the hypothalamus and dependent on serotonin-thyroid hormone interactions. Data in support of this hypothesis include: imbalance in central nervous system (CNS) neurotransmitters with ageing, with serotonin dominance; impairment of hypothalamic neurosecretion; increased heterogeneity of thyrotropin forms secreted from the aged pituitary; and drug and dietary manipulation of CNS serotonin with consequent effects on endocrine function and homocostasis.

Data are presented relating to the nutritional status of over 1500 elderly subjects seen in three community surveys. There was a decline in weight, arm circumference and skinfold thickness, and to a lesser extent in Quetelet's index (weight÷height2), with advancing age. Haemoglobin levels declined with age in one area but not in the others, probably due to differences in nutritional state. Plasma protein and albumin concentrations showed no decline with age and were similar to values reported from younger subjects. Men who ate alone tended to have a poorer vitamin C and thiamine status than men whose meals were cooked or shared by another person, but no such difference was found among women; indeed, women eating alone had significantly higher leucocyte ascorbic acid levels.

Anthropometric data (triceps skinfold thickness, mid-arm circumference and arm muscle circumference) have been obtained from 114 normal elderly individuals and are presented as a suitable reference range for use in nutritional assessment of the elderly. In males the results did not differ significantly from existing standards for younger men but in females there was a significant difference from similar standards for younger women. These findings suggest that there is an increase in fat and a decrease in lean body mass with advancing age in females but not in males. This difference is discussed.
